If we’re a good match, I will email you a link to complete an online referral form, or we can complete it over the phone. If we’re not the right fit, you can continue your search via Occupational Therapy Australia’s Provider Search.

Do I need to have a Free Call to be supported by EOT?

No, but it certainly helps! If you’re comfortable with the information you’ve read and want to get started, send me an email requesting a referral form (NDIS, Medicare or Private) and I’ll email you a link to complete online.
